Thomas Huth 78751ea855 Remove QEMU_ARTIFICIAL macro 
The code that used it has already been removed a while ago with commit
dc41aa7d34 ("tcg: Remove GET_TCGV_* and MAKE_TCGV_*").

Paolo Bonzini 3f7a899ff4 add macro file for coccinelle 
Coccinelle chokes on some idioms from compiler.h and queue.h.
Extract those in a macro file, to be used with "--macro-file
scripts/cocci-macro-file.h".
