If you're using a program like JoyToKey, this may interfere with reading the controller.
The ***Steam*** application has been known to also surreptitiously change the controller buttons around causing a button you press to register as a different one. Quitting Steam or turning off the option to remap controllers inside Steam will fix the problem.
