All known-dumped disk sides are now included. A number are still missing and need dumping, however.

Fix some disk naming formatting I'd missed as well.

I'm not entirely sure about src/mame/drivers/nova2001.c line 933,
as it could plausibly be either "which is not the top bit" or "which
is now the top bit", with almost opposite meaning.
